2018-2019 Schedule Pick Up Day Information & Forms
Thursday, August 16, 2018
8:00 - 10:00 a.m. (6th grade Adventure Island students only)
10:00 a.m. - 3:00 p.m. (all Smith students)
Fill out all applicable forms in advance of registration (see below).
Also, please be sure to check your email. Every family will be receiving an email from our district with information on how to complete their online emergency card. The email will detail how to reach the website, how to log in, and then complete the emergency card. Please be sure to provide up to date contact information including work numbers and cell phone numbers to reach you throughout the day. In addition, please be sure to provide full names and details for any other family members or adults that may be granted permission to also pick up your child from school.
Online Emergency Cards MUST be completed before your student will receive their schedule.
STATION 2 - IPad Pickup
At middle school registration on August 16, students will receive their iPads. Prior to registration, please review and complete the on-line Student Responsibility Form located at: www.tsdtechforms.com A separate form is required for each student. Any student that does not have a form on file by September 13, 2018, will be required to turn in their iPad until the form is received.
STATION 3 - Smith PTO and Organization Donation Forms
(Thank you for donating to the Smith PTO and other organizations)
Smith PTO is accepting donations via credit card.
STATION 5 - Picture Order Form

STATION 7 - Smith PTO Shop-n-Stop school store and spirit wear orders
(Bring $ to purchase school supplies; spirit wear samples on hand for sizing)
